California Department of Health Care Services Technical Assistance Marketplace
At VitalPath, we led the successful implementation of digital health transformation projects under the California Department of Health Care Services (DHCS) TA Marketplace initiative. Serving as the strategic project management partner, we supported public health agencies and community-based organizations (CBOs) in adopting scalable, technology-enabled solutions aligned with CalAIM objectives. Justice-Involved Reentry Initiative
VitalPath played a key role in supporting the implementation of the CalAIM Justice-Involved Reentry Initiative, a statewide effort to improve continuity of care for Medi-Cal members transitioning from correctional facilities. As a subject matter expert, we provided strategic consulting and implementation support for counties, correctional facilities, managed care plans, and community based organizations. Our team developed comprehensive workflows and readiness plans to ensure seamless reentry care coordination. We authored business requirements documents, training materials, and compliance frameworks to support program launch and sustainability. Additionally, we established key performance indicators and risk mitigation strategies to ensure accountability and continuous improvement. Behavioral Health Quality Improvement Program (BHQIP)
VitalPath supported thirty-two of California’s counties in achieving the state’s Behavioral Health Quality Improvement Program (BHQIP), a $2.2 billion statewide initiative aimed at strengthening behavioral health systems across counties. Our team provided end-to-end project management, including the development of detailed project plans, data dashboards, and reporting tools to ensure transparency and accountability. We coordinated intercounty teams and engaged executive stakeholders to align efforts with state objectives. Gestational Diabetes Public Health Program – Peru
VitalPath designed and implemented a culturally responsive public health program to address gestational diabetes mellitus (GDM) in underserved communities in Cuzco, Peru.
